5100 Camden Ave, San Jose

Cost: $75 for the first student and $60 for each additional student (from the same family).

Dress Code:

Young Ladies: Modest, no strapless dresses and no cleavage. Skirts and dresses must be a modest length.

There will be an outside patio so girls should bring a sweater or shawl.

Young Men: Dress slacks, dress shirt, and tie. Suit jacket preferred. No tennis shoes.

By participating in the Live Oak Formal Dinner and Dance, I acknowledge that my child may be photographed, videotaped or otherwise recorded when using the chocolate fondue fountain or other activities at the dance.
I have reviewed the dress code with my student(s) and agree to follow it:

The DJ will NOT be taking song requests the night of the dance.
I will pay Live Oak Academy $75 for my first student and $60 for each additional student (from the same family) attending the dance using the "Add to cart" buttons available after I submit this form.
