Student application


This form should be completed by families applying for the 2018-2019 Academic year.

Before submitting your online application, please read the following information carefully and review the additional documents that are requested as part of the application package for your student’s application.

Application deadline and non-refundable fee

$225 per student
Application fee goes up after April 30

The application fee must be paid online when application is submitted. The application will not be reviewed until our office has received the application fee.

Application package

In addition to the application form and application fee, the following additional documents must be submitted to the Academy in order for your student’s application to be complete.

Gather as many items from this list that are available for your student:

  1. A copy of the most recent standardized tests taken (no earlier than the last two years).
  2. A transcript or grade report for the past two years.
  3. A reading list for the past one year (for grades 2 – 12).
  4. Two writing samples from the past one year (for grades 2 – 12).
  5. A Family photo with the student applicant circled (a photocopy is okay).

Important:  Turn in only single-sided copies. The student’s name must appear on each item. 

The application package can be submitted in one of three ways:

  • Brought to the Academy office at 2499 Homestead Road.
  • Scanned and e-mailed to Admissions@liveoakacademy.org.
  • Sent to the Academy mailing address (different than our campus address):
