HSLDA Group Discount Program
Live Oak has applied to HSLDA as a Discount Group, allowing all members
to receive HSLDA membership at the discount rate of $95. The
normal price is $115.
• If you are already a member, please fill out this
enrollment form and turn it in to our Academy office.
• If you are not yet a member, please fill out the standard HSLDA
application form and mail the application form to HSLDA along with
a check for $95 payable to HSLDA.
• If you are a new Live Oak Academy family and have already
mailed your HSLDA membership application as part of your Live Oak
Application process, we'd like to
apologize to you for missing this opportunity, by giving
you a one-time $15 tuition credit. Just submit the same enrollment
form to us as long-time members, and write
"RECENT APPLICATION" in the top margin.
Our group membership discount rate is applicable as long as we maintain
15 Live Oak Academy member families in our group.
